​​​​​​​​​​学内での定期健康診断を受けられなかった学生は、①大学の提携医療機関での健康診断を受けることができます。
下記の要領をよく読み受診してください。
本学からの健康診断証明書が必要な場合は、学内での定期健康診断もしくは①の健診を受ける必要があります。

大学主催の健康診断を受けられない方は、自分で外部の医療機関で健康診断を受けてください。
外部の医療機関で健康診断を受ける／受けた場合の説明は、ページ最下部②に記載の通りです。

① 大学の提携医療機関での健康診断（予約制）

混みあう時期は希望通りの日程で予約が取れるとは限りません。
早めに予約をしてください。

なお、特別卒業・特別修了を希望している方の受診期限は8月17日（土）までです。それ以降に健診を受けても無効です。                                              早めに手続きをしてください。

予約期間：

2024年4月15日（月）～12月23日（月）　

実施期間：

2024年4月15日（月）～12月26日（木）

予約場所：

在籍キャンパス保健室

予約方法：

「学生証」を持参し在籍キャンパス保健室に来てください
健診に持参するものを渡す必要があるので、電話やメールでの申し込みは受け付けていません

実施場所：

市ヶ谷にある提携医療機関

料　　金：

健診場所までの交通費は自己負担です。健診料金は無料です。

本学からの「健康診断証明書」について

和文1通300円、英文1通400円

「健康診断証明書」発行が可能となるのは、①の健康診断を受けてから2～4週間後です。
再検査や診療が必要となった場合は発行が遅くなることがあります。

② 外部の医療機関での健康診断

「学内での定期健診」もしくは「①の健診」を受けられず、外部の医療機関で健康診断を受けた場合、その結果や診断書のコピーを保健室へ提出すれば、本学の健康診断を受診したものとみなすことが出来ます。
この場合健診済印*は押印できますが、大学からの健康診断証明書は発行出来ません。
こちら②の健診費用は自己負担です。

社会人の方で、勤務先・自治体・その他医療機関等で2024年4月1日以降に健康診断を受診した、又は受診する予定がある場合は、結果のコピーを提出すれば本学の健康診断を受診したものとすることができます。
既に受診した場合は2024年5月末までに、又、これから受診する場合は随時健康診断の結果（コピー可）を保健室にお持ちいただくか、郵送またはメール添付で送付してください。

• 体育の授業を履修するとき、PRAC・SPACのプールを利用する際などに必要になります。通学定期乗車券発行控に押印します。
  

健診項目

学年ごとの項目は以下の通りです。必要時、診断書フォーム（PDF）をご利用ください。

(1) Health checkup at an affiliated medical institution (reservation required)

Reservation period :

April 15, 2024 (Mon.),- December 23, 2024 (Mon.)　

Checkup period :

April 15, 2024 (Mon.) - December 26, 2024 (Thu.)

Reservation location :

Affiliated campus Health Center

How to make an appointment :

Bring your "student ID card"  to the Health Center to make an appointment.

Fee :

Free.
Travel expenses to the location where the health checkup is being held will be borne by the student.

 "Health check certificates" issued by the University

Japanese: 300 yen per copy, English: 400 yen per copy

"Health check certificates" can be issued from two to four weeks after receiving a type (1) health checkup.
Issuance may be delayed if re-inspection or a medical examination is required.

(2) Health checkup at another medical institution

If you submit the results and a copy of the medical certificate to the health center, you can get a stamp on your "student commuter pass certificate" to certify that you have completed the checkup, but we cannot issue a "health check certificates*" from the University.
In this case, you are responsible for the cost of the health checkup.

• "Health check certificates" is required when taking sports classes, using the PRAC/SPAC pool, etc.
